If you wish to apply for the certificate online, you can do so but if you wish to apply offline then check marriage registrar office and visit the office personally.
Fill Marriage Registration Form and upload all necessary documents. After that choose your Date of Appointment to Registrar Office and submit the form. Save acknowledgment slip. Now on the given date you will have to be presented before the ADM with original documents and witnesses and spouse.

To apply offline for marriage certificate, visit sub-registrar office. To locate registrar office, you can check official website. Get a copy of application form of Marriage certificate. Download the application form from the official website and take a print out. Fill it with appropriate details. Here name of the bride can be changed with new name.
